YOUR CONTRIBUTION WILL
HELP FAMILIES IN NEED Donate Online or Download Mail-in Contribution Card Thank you for considering a contribution to support the work of the Alzheimer’s Association. Thousands of families in Pennsylvania know first-hand that the resources and materials of the Association provide valuable information, support groups and educational programs to better cope with the impact of this and other devastating dementia diseases. Your donation to the Alzheimer’s Association, Greater PA chapter, may be made through your contribution of cash or appreciated stock, as well as through the donation of life insurance policies or gift annuities. A contributions card follows this information and may be printed and mailed with your donation to the address listed on the form. Many employers have matching gift programs, which doubles your contribution, please check with your employer. If you prefer, a donation to support our work may also be made directly on-line through the national Alzheimer’s Association, http://www.alz.org/Donate/overview.asp You may decide to further the work of the Alzheimer’s Association, Greater PA chapter, with a contribution to be paid from your estate. This donation may be either a designated amount or a percent of the remaining estate. We suggest that long term financial planning take place with your tax or financial advisor or your attorney. We hope that you will consider a bequest or planned gift as this contribution can be a simple way to assure that the support, resources and education and that you, your loved one, and your family have received during your lifetime will remain available for families yet to need these services. In many cases, a simple amendment can be made to your will, and language may be added without re-writing the document. We encourage you to speak with your advisors about making a planned gift from your estate. As required, the official registration and financial information of the Alzheimer’s Association, Greater PA chapter, may be obtained from the Department of State, Commonwealth of Pennsylvania, by calling toll-free with PA, 1-800-732-0999. Registration does not imply endorsement. One of the best and easiest ways to help in the fight against Alzheimer’s Disease can be summed up in a word: Volunteer!
